Please complete the forms below and submit them by clicking both yellow buttons at the bottom of this page. Be sure to send the completed forms to both Marni and Lisa. It’s important that these forms are submitted promptly, as they are required for you to receive your first paycheck on time. Alternatively, you can drop them off at the office to be entered into our ADP payroll system. Payroll is every two weeks. Please be aware that there is a nine day processing period prior to receiving your first paycheck. The pay period will be indicated on your check for reference.

Currently, we do not offer direct deposit. If you prefer your check mailed, please notify the office. Otherwise, it will be available for pick-up at Homework Hub.

The following forms are included:

W-4 Form (Federal Withholding)

IT-2104 (State Withholding): I will assume the same withholding amount as the Federal unless specified otherwise on this form. I only need you to return the first page, the others are calculation worksheets for your reference.

I-9 Form (Employment Eligibility Verification): Please fill out and return only the first page and provide a photocopy of an acceptable identification as per the chart on the second page.

** Please make sure to include your email address on the I-9 form so that I can add you to our email group for payroll reminders.

New York Paid Family Leave Waiver: This is a new withholding effective January 1, 2018 and will automatically apply to your wages UNLESS you complete the waiver form. Please note that you have to qualify to opt out, so please read the instructions carefully that go along with this form.
