Priority Form - Students

HwH Priority Communication Form


Please prepare for your child's session:

1. Due to the large volume of communication the HwH receives daily, please send a text to 516-423-2543 letting us know a priority communication form has been submitted.

2. Make sure your child brings all necessary materials (agenda, workbooks, folders, textbooks, calculators, computer, etc.).

3. Remember to submit this form before dropping off your child. If not, our teachers will use their best judgment.

What digital platform is your child's school using (Microsoft Teams, Canvas, Google Classroom, etc.)?

OPTIONAL BUT STRONGLY RECOMMENDED: If you feel your child won’t be able to access their digital platform on their own: What is your child’s log in information?

(Please keep in mind how long your session is when creating your list. A teacher can only cover what your child can grasp in a reasonable amount of time).
